[Maillist] Alternative School Application

Greetings from Vinton-Shellsburg!!

 

I am in the process of getting our new Alternative Program off the ground
and am looking for a little help with something.  We have had a surprising
number of students that are interested in attending the program next year.
We would like to have them fill out some sort of application that we can
look through and use to determine who gets in and who is on the waiting
list.  Do any of your schools or programs have an application like this?  If
so, could you send me an electronic version of it to look at?  I'm just
looking for ideas and curious as to what everyone else uses.  I know it is
an incredibly busy time of year and I apologize for that, but any help you
could give me would be extremely appreciated.
